About Job
- Completing assessments (admission assessments for rehab and long-term care, depression screening, trauma screening)
- Coordinating services for discharge planning
- Attending and assisting with team/family meetings and care planning
- Providing emotional and social support for residents and families
- Currently enrolled in a Bachelor's or Master's degree program in Social Work, Human Services or a related field.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
- Ability to maintain confidentiality and handle sensitive information with professionalism
- Pay Range $0.01 - $0.01 / hour
