This role involves completing psychosocial assessments to evaluate patient needs, supporting patients and families through hospitalization and diagnoses, and providing crisis intervention and grief counseling.
- Collaborates with care teams, including specialists, to develop treatment plans.
- Maintains a database of community-based psychosocial resources for patients and staff.
- Requires a Master’s in social work (MSW), knowledge of CPS and APS laws, and preferably 2+ years of experience in social services or discharge planning, with strong preference for palliative care experience.
- Must have excellent communication skills, basic medical terminology knowledge, and proficiency in Microsoft Office and electronic medical records.
- No specific licensure required.
- Serves all ages, and offers a salary range of $54.18 - $72.04/hour.
- The position is primarily sedentary, with occasional lifting and physical activity, and accommodations are provided for qualified individuals.
