Sub - Care Manager - Mental Health Urgent Care
Provide case management and support services to adults seeking mental health support in an alternative setting to the Emergency Department. This position offers a combination of administrative responsibilities and direct service. Great opportunity to support individuals in a community-based setting.
Sub Care Managers cover open shifts due to vacancies and planned/unplanned time.
Responsibilities:
- Provides direct services and support to clients; determines quality interventions, maximizing resources and fostering the clients' independence
- Care coordination, including communication with internal and external providers and professionals in other disciplines; schedule and attend team meetings
- Completes referrals to other services, community resources and different levels of care
- Establishes relationships and partnerships with local resources
- Completes agency and program documentation and reporting
- Participates in staff meetings, group and individual supervision, case staffing, training and other program administration
- Assist clients with crisis prevention, de-escalation and conflict management
Requirements:
- A combination of education and relevant experience
- Valid driver's License and use of a personal vehicle
Structure:
- Hours vary
- Non-Exempt
- Hourly rate of $23.45
