Role Overview
As part of a dedicated team, the Care Transition Manager plays a crucial role in ensuring seamless discharge planning for plan members who visit the Emergency Room. You will utilize AmeriHealth Caritas policies and procedures to conduct thorough multidisciplinary assessments and make essential case management referrals.

Work Arrangement
This position is on-site at Mercy Fitzgerald Emergency.

Responsibilities

• Conduct daily reviews of ER patients, manage facility referrals promptly.
• Engage with plan members face-to-face and administer surveys to determine their unique needs.
• Identify any care gaps and evaluate members' understanding of their conditions and outpatient management, alongside coordinating discharge needs across various disciplines such as primary care providers, home health services, skilled nursing, rehabilitation services, durable medical equipment, pharmacy needs, and social work referrals.
• Schedule necessary appointments and facilitate critical follow-up communications to enhance care coordination and educate members on accessing appropriate care and services post-discharge.

Education and Experience
- Associate Degree in Nursing
- A minimum of 3 years of nursing experience in a clinical setting is required.
- Managed care experience is preferred.
- Experience in Emergency Room or Acute Discharge Planning is highly valued. A strong background in Behavioral Health is especially preferred.

Licensure
Registered Nurse.
