What You'll Do

Provide skilled home health Medical Social Work care: Perform in-home assessments: Evaluate geriatric patients' living situations, health, and resources.
Connect to vital resources: Secure community aid like IHSS and Meals on Wheels for patients.
Guide IHSS processes: Assist with applications and modifications.
Document efficiently: Use our simple EMR system for all notes.
Collaborate: Work with a supportive care team, ensuring coordinated care amongst supervising therapists, other disciplines, and within our administrative team to optimize patient outcomes.
