Care Coordinator

Location: Elgin, Illinois

Salary: $20–$26/hr (based on experience), bonuses

Position Overview

We are seeking a highly motivated and compassionate Care Coordinator who can wear multiple hats, coordinate caregiver schedules and actively seek and onboard new clients. This is a critical role for someone who is passionate about home care and will ensure our clients receive the highest standard of support.

Key Responsibilities

*Coordination

· Match clients with appropriate caregivers, ensuring availability, skills, and compatibility.

· Maintain and manage the caregiver schedule, including handling changes, call-offs, and coverage gaps.

· Ensure compliance with agency procedures and Illinois non-medical home care regulations.

*Sales & Client Acquisition

· Actively seek new clients through outreach, community engagement, referrals, and partnerships.

· Conduct intake consultations and present service offerings to prospective clients and families.

· Build and maintain relationships with hospitals, senior living communities, clinics, and local organizations.

· Track leads, conversions, and client satisfaction.

Qualifications

· High school diploma or equivalent required; associate or bachelor's degree preferred.

· At least 2 years of experience in caregiving, scheduling, or coordination within a home care or healthcare setting.

· Previous sales, intake, or community outreach experience is highly preferred.

· Knowledge of non-medical home care regulations in Illinois.

· Excellent written and verbal communication skills.

· Tech-savvy – must be comfortable using scheduling software and mobile apps.

· Reliable transportation with valid driver’s license and insurance (travel required).
