Keystone Home Health is seeking an experienced home health intake coordinator to join our team. We offer competitive pay and benefits package.
The Clinical Intake Coordinator is responsible for the timely and accurate completion of the referral process and the initial coordination of information to clients, their family members, physicians, referral sources, facility staff, agency staff, and any others involved in assuring continuity of client care.
QUALIFICATION GUIDELINES
EXPERIENCE/TRAINING/EDUCATION/CERTIFICATION:
· Minimum of one year experience in home care, hospice, or personal care services, preferred
· Prior use of Electronic Medical Records (EMR), preferred
· Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payors, preferred.
KNOWLEDGE/SKILLS/ABILITIES:
· Customer service focused, including the ability to set the caller at ease and encourage open dialogue about the client’s needs
· Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client Intake
· Ability to maintain composure in addressing stressful and/or challenging situations
· Excellent telephone, verbal and written communication skills, with the ability to communicate with both professionals and non-professionals
· Excellent organizational skills
· Detail oriented
· Knowledge and skills in the use of computers and software programs
· Knowledge of pertinent state, federal, and local regulatory requirements
Job Duties (but not limited to):
1. Provides initial coordination of information with clients, their family members, physicians, referral sources, facility staff, agency staff, and any others involved in assuring continuity of client care.
2. Informs referral sources, clients, family members, and physicians about
clinical and non-clinical programs available in the home setting.
3. Ensures referral orders are appropriately documented in the client’s medical record.
4. Collaborates with the client, physician, referral source, sales, and director to collect information needed to process a referral.
5. Provides information to the branch director and sales team to resolve aging referrals.
6. Provides information to the branch director and sales team that will assist in determining non-admission decisions for pending referrals.
7. Documents all referral related information, actions taken, and follow-up required in the medical record in a timely and accurate manner.
8. Other duties as assigned.
