Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Primary Purpose of the Position is: to provide complex social work services to adult and geriatric populations hospitalized in an acute medical, surgical and/or critical care unit. The Inpatient Complex Case Senior Social Worker is a part of the medical, surgical and critical care units which delivers inpatient care to Veteran patients in the acute inpatient setting. The Senior Social Worker functions as a discharge planner with advanced skills meeting the needs of Veterans with complex care and psychosocial needs, and coordinates care with a multidisciplinary team to ensure that discharge planning needs are in place prior to the Veterans discharge from the inpatient setting. The Senior Social Worker demonstrates an advanced knowledge of social work practice, advanced application of social work principles, understanding of sound program management practices, and prosocial peer relationships. Must be licensed at the advanced clinical level, including the licensure necessary to clinically supervise dependent social workers for the purposes of licensure.
Works with patients and families who are experiencing a variety of psychiatric, emotional, medical, and social problems utilizing individual, group, and family counseling skills. Independently provides complex case management, treatment planning, goal setting, crisis management and clinical interventions providing discharge planning with more complex patients. Assesses the psychosocial functioning and needs of patients and their family members, and partners with the patient and caregivers to formulate and implement a treatment plan, identifying the patient's and their family members, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.
Identifies high-risk patients and provides case management services. Senior Social Worker provides crisis intervention services, seeking to address the cause as well as the resenting complaint, coordinates family conferences and serves as liaison to family members
Independently provides consultation and guidance to colleagues regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Evaluates own practice through professional peer review, case conferences and other organizational means.
In case of antagonistic, indifferent, or evasive Veterans and their significant others, constructively works with both positive and negative reactions, understands disparate values for Veterans and families from different social and ethnic groups
Independently evaluates Veterans and/or caregivers/significant others and performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Develops a biopsychosocial assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family, caregivers, and/or significant others, as appropriate.
The Senior Social Worker may serve on committees, councils, workgroups, and/or task forces and may serve as liaison to community agencies and networks with agency representatives

Basic Requirements:
Grade Determinations: Senior Social Worker GS-12
Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Examples of specialized experience include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

Preferred Experience:
- 2 years of Medical Inpatient or Outpatient Social Work Experience
- 2 years of experience conducting psychosocial assessments
- Experience providing case management and discharge planning in complex discharges and addressing complex psychosocial factors
- Experience making referrals for community resources such a, but not limited to nursing facilities group homes, assisted livings, substance abuse treatment, and hospice services.
