About the Role
We are a compassionate hospice care provider seeking a Licensed Vocational Nurse (LVN) to join our team as a Patient Care Coordinator. This primarily office-based position focuses on coordinating all aspects of hospice care, including ordering medications and supplies, supporting field staff, and maintaining smooth, timely communication across the care team. Occasional field visits may be required for patient change of condition assessments.

Key Responsibilities

  • Coordinate and manage care plans for hospice patients in collaboration with the IDG
  • Order and monitor medications, DME, and medical supplies to ensure timely delivery
  • Serve as the central point of communication for physicians, nurses, aides, social workers, and other care team members
  • Triage patient needs and support field staff with urgent care coordination
  • Document all updates, communications, and interventions accurately in the EMR
  • Participate in IDG meetings and ensure compliance with hospice regulations
  • Provide family education and support via phone and, occasionally, in person
  • Perform occasional field visits as needed for patient change of condition assessments

Qualifications

  • Active and unrestricted LVN license in the state of California
  • Hospice experience strongly preferred
  • Case management or care coordination experience is a plus
  • EMR proficiency required (HospiceMD experience a plus)
  • Excellent organization, multitasking, and communication skills
  • Ability to thrive in a collaborative, fast-paced environment
  • Valid driver’s license, proof of auto insurance, and reliable transportation (for occasional field visits)
  • BLS (Basic Life Support) certification required
  • Minimum 1 year of clinical experience preferred
  • Excellent organizational, multitasking, and communication skills
