The primary purpose of this position is to assist the Director of Social Services in planning, organizing, and directing the day-to-day functions of the Social Services department to ensure that residents’ psychosocial needs and discharge planning processes are effectively managed, as directed by the Administrator or the Director of Social Services. This role supports Aliya Healthcare’s mission to promote high-quality, compassionate care in every facility we serve.

Role Description

This is a full-time on-site role for a Discharge Planner, located in the Greater Chicago Area. The Discharge Planner will be responsible for coordinating and planning patient discharges, managing cases, and ensuring patients' needs are met upon leaving the facility. Daily responsibilities include collaborating with healthcare teams, communicating with patients and their families, and managing utilization to ensure efficient and effective care transitions.

• Experience in Discharge Planning and Case Management
• Strong Communication skills for collaborating with healthcare teams and engaging with patients and families
• Knowledge in Social Work and Utilization Management
• Excellent organizational and time-management abilities
• Ability to work collaboratively in a healthcare setting
• Familiarity with healthcare regulations and standards
• Bachelor's degree in Social Work, Nursing, or a related field
