JOB SUMMARY

A Master Level professional in social work or counseling, responsible for providing direct care services to a team of individuals considered to be part of the Williams Assertive Community Treatment Team. The clinician will be responsible for providing offsite and onsite individual, group, and family therapy as well as family psycho-education services and complete all clinical documentation to maintain clients authorization for mental health services (documentation including but not limited to IM+CANS, Treatment Plans, Suicide Assessments, Crisis Plans)

JOB DUTIES

  1. Must be available, as rotation determines, on a 24-hour basis, to intervene to crisis needs. Must carry crisis phone for 24 hours daily
  2. Provide billable Medicaid services to all assigned clients; totaling 30 hours per week
  3. Ensure that all required reporting/data are submitted to the Department of Mental Health within specified timelines.
  4. Maintain advanced knowledge of Rule 132, Attachment B, and all regulatory standards as it relates to programs within the Division of Mental Health;
  5. Ensure that all charts are within compliance with all regulatory standards including DMH and CARF.
  6. Completes Mental Health Assessments through a formal process of gathering information regarding a client's mental and physical status and presenting problems resulting in the identification of the client's mental health service needs and recommendations for service delivery;
  7. As a member of an interdisciplinary team develop comprehensive Treatment Plans within the required timeframes;
  8. Assure the participation of team members, the individual, the guardian, the family, and necessary; professionals in the treatment process, unless the individual is not legally disabled and does not desire the involvement of the family or the family refuses to participate;
  9. As indicated and within the confines of confidentiality counsels family members to assist them in understanding, dealing with, and supporting clients.
  10. Meet with families, probation officers, police, and other interested parties to exchange necessary information during the treatment process.
  11. Refer patients, clients, or family members to community resources or to specialists as necessary.
  12. Initiate and coordinate the interdisciplinary process as often as specified in the services plan or when required by problems or changes;
  13. Provide intensive stabilization, assessment, crisis intervention, and case management;
  14. Participate in all required training as offered in the mental health division;
  15. Ensure that client records are maintained in accordance with Section 115.300(I);
  16. Provide individual, family, and group therapy to assigned clients to promote emotional, cognitive, behavioral or psychological changes as indicated;
  17. Guide clients in the development of skills and strategies for dealing with their problems;
  18. Prepare and maintain all required treatment records and reports;
  19. Evaluate the effectiveness of current counseling interventions and clients' progress in resolving identified problems and moving towards defined objectives.
  20. Modify treatment activities and approaches as needed to comply with changes in clients' status.
  21. Learn about new developments in their field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other social service agencies.
  22. Actively discuss with individual clients their plans for life after successful discharge from the program.
  23. Gather information about community mental health needs and resources that could be used in conjunction with therapy.
  24. Collaborate with designated county agencies to ensure consumers’ stability in the home;
  25. Assist, guide, and refer consumers to other resources, as needed.
  26. Maintain confidentiality of records relating to client's treatment;
  27. Perform other duties as assigned

QUALIFICATIONS

  • Master Degree in related to social services, counseling, or human services field
  • Valid Driver’s License and vehicle required to travel
  • Sensitivity to multi-cultural differences
  • Ability to relate well to people of all socioeconomic groups
  • Knowledge of languages other than English helpful, particularly Spanish
  • Undergraduate Degree in Social Work/Psychology or an appropriate human service field;
  • The candidate must have a solid understanding of the DSM IV-TR criteria for diagnosing Mental Health / Substance Abuse patients and a solid understanding of the Illinois Mental Health Code.
    Knowledge of Community Mental Health, Rule 132 and the Department of Mental Health;
  • Must have the ability to formulate the 5 Axis diagnoses and formulate the initial treatment plan.
  • Must be able to ascertain the client's presentation of illness, evaluate the patient's functional level prior to admission and determine the client’s level of risk.
  • Knowledge of and proficiency in computer programs, i.e. Microsoft Word, Excel, Adobe, etc;
  • Demonstrate a high degree of skill in problem-solving and working effectively with people.
  • Must be detail-oriented and flexible.
  • Ability to communicate effectively and serve as a liaison to primary referral sources, families, team members, school districts, community agencies, and signatory partners.
  • Must have good communication skills and team relations to provide accurate handoffs in acute situations.

WORKING CONDITIONS AND PHYSICAL EFFORT:

  • Work is normally performed in a typical interior/office work environment.
  • Moderate physical activity. May require physical effort including lifting up to 15-25 pounds and some extended periods of standing or walking.
  • No or very limited exposure to physical risk.
  • Ability to lift 15 - 25 lbs.
