Job Title: Medical Social Worker (MSW) – Hospital Discharge Planning
Location: Holy Cross Hospital, Silver Spring, MD
Job Type: Contract | Onsite | 40 hrs/week
Schedule: Monday–Friday, 8:00 AM – 4:30 PM

Pay Rate: $44.55/hr (includes taxable base + per diem)

Start Date: August 17, 2025 (3 months with possible extension)

Job Description:
Seeking an experienced MSW to support discharge planning for adult, pediatric, and maternal patients. Role includes admission assessments, care coordination, and collaboration with interdisciplinary teams. Must have 3+ years of recent acute hospital discharge planning experience.

Requirements:

• Active State Social Work License
• BLS Certification
• Recent acute care experience (3 years)
• Skills checklist & 2 professional references

Job Type: Part-time

Pay: $44.55 per hour

Expected hours: 40 per week
