About the Role:

As a Care Manager, you’ll be the key point of contact for care recipients and their families.

You’ll lead the charge in managing client relationships, supervising caregivers, and coordinating care to ensure everyone feels supported and well cared for.

You’ll work closely with an exceptional team to address daily needs, ensure compliance, and contribute to creating a culture of kindness, respect, and compassion.

What You’ll Do:

● Conduct in-home assessments and create personalized care plans

● Build strong relationships with clients, caregivers, and families.

● Match caregivers with clients based on their needs and our staffing philosophy.

● Supervise and support caregivers through feedback, evaluations, and regular

communication.

● Ensure compliance through accurate documentation and regular home visits.

● Collaborate with your team to coordinate services and triage emerging needs.

● Participate in a rotation for after-hours weekday and weekend support.

What We’re Looking For:

● Excellent communicator with strong people skills.

● Highly organized, reliable, and proactive.

● Computer savvy and able to document efficiently.

● Able to think on your feet and stay upbeat under pressure.

● Valid driver’s license and reliable transportation.

● Experience as a caregiver or in a supervisory role is a plus!

Compensation & Perks:

● Hourly Pay: $27–$32/hour with bonuses

● Paid Time Off: 10 days per year

Schedule: Full-time; rotating evenings and some weekends as needed
