Position Description – Clinical Care Coordinator/RN Care Manager

Location: Ocala, Florida
Schedule: Part-Time | In-Person

Evergreen Private Care is seeking a compassionate and organized Registered Nurse (RN) to join our team. This role offers a unique opportunity to blend clinical care with case management, supported by a collaborative team.

What You'll Do:

• Complete initial and ongoing in-home assessments to identify clients' physical, psychosocial, and environmental needs
• Develop and manage individualized care plans in alignment with client goals and clinical standards
• Serve as a liaison between the clinical team, caregivers, schedulers, and families
• Participate in weekly team meetings, documentation, and client service planning
• Assist in training and conducting skills assessments for new caregivers
• Participate in rotating after-hours clinical on-call coverage as needed)

Qualifications:

• Active Florida RN license in good standing
• Home health, hospice, or case management experience preferred
• Strong clinical judgment and organizational skills
• Excellent communication and documentation abilities
• Comfortable with both field and office-based work

Job Type: Part-time

Pay: $22.30 - $26.85 per hour

Expected hours: 15 – 30 per week
