Care Resources is hiring an Aging Life Care Manager in the Tampa area to complete our experienced team of experts in Aging, Mental Health and Chronic Illness. A Bachelor's degree in Gerontology, Social Work, Nursing or relevant Human Services field is a must! Master's Degree preferred. Remote Client assignments in Pinellas County, with occasional assignments possible in Pasco and Hernando counties.

A Care Resources' Aging Life Care Manager will provide a variety of services including, but not limited to:

1. Preparing a comprehensive psychosocial assessment with recommendations;

2. Developing a personal care plan;

3. Coordinating and monitoring of services from ancillary service providers;

4. Acting as a Client advocate;

5. Assisting with benefits applications, when necessary;

6. Assisting with relocation, when necessary;

7. Attending medical appointments and monitor the Client's status.
