Job Details:

Full Time Direct Hire Opportunity
Schedule: Monday-Friday Hybrid Roll 3 days in office to start, possible 2 in the future
Must be LCSW not LMFT
Sessions are typically 1hr to half hour
12 sessions per day- visits are monthly- in person and video/hybrid visits.
Patient Population- Diverse Patient population priority focus on adults will work with children and adolescents as well
Provide comprehensive social work services in a high-census environment
Conduct psychosocial assessments and develop treatment plans for patients
Collaborate with interdisciplinary teams to ensure holistic patient care
Connect patients with appropriate community resources and support services
Advocate for patients' needs and rights within the healthcare system
Document patient interactions and treatment progress according to facility standards
Participate in care coordination meetings and treatment planning
Locations are FQHCs or CCBHC and all considered Health Professional Shortage areas

Compensation: Competitive salary with full comprehensive benefits. Salary $92-122K/annually DOE. Eligible for state and federal loan repayment programs.
