The Care Coordinator RN will be a central point of contact for patients, managing their care and coordinating various aspects of their healthcare journey. This person will work with patients, their families, and the healthcare team to ensure effective communication, smooth transitions between care settings, and optimal patient outcomes.

• $95K - $120K depending on experience

• Days, Full time

Required Qualifications:

• Current RN license in Alaska or Multistate

• Case Management certification within 24 months of hire.

• BLS, ACLS

• 5 years clinical nursing experience
