FT MEDICAL CARE COUNSELORS – Residential Services for Developmentally Disabled Adults

Copiague, LI (6a-2p, Mon.-Fri.) – Starting pay rate: $23.00/hr.

Responsibilities: The role of the Medical Care Counselor is to support and provide health-related advocacy for individuals residing in our group homes for developmentally disabled adults.


Key activities include:

  • Providing continuous protective oversight to the individual
  • Scheduling, transporting and accompanying individuals to appointments with providers; facilitating communication with providers re: individual’s health changes
  • Maintaining all health-related documentation, including lab work results, consultation reports and updated medication prescriptions
  • Communicating daily with nursing and residential management re: individual’s medical status and treatment plans
  • Managing medication inventory including prescriptions, over-the-counter medications, nutritional supplements and first aid supplies
  • Providing companionship visits to individuals that are hospitalized, as required

Qualifications:

  • HS diploma or GED
  • Experience working with Developmentally Disabled Adults preferred
  • N.A. or equivalent training is helpful
  • Valid NYS Driver’s License
  • Ability to transfer and transport individuals to/from medical appointments in all types of weather, using house passenger van.
  • Detail-oriented, with well-developed organizational and interpersonal communication skills
  • Willing and able to provide compassionate, “person-centered” care

This rate of pay serves as a good faith estimate. Actual pay is based on a number of factors which may include a candidate’s qualifications, skills, competencies, experience and work location. The salary range or rate listed does not include any differential pay or other forms of compensation, such as the value of benefits, which may be applicable to this position.
