We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for Vocational Rehabilitation Counselors who want to provide truly patient-centered care in Denver, CO, and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Collage Home and Community Rehabilitation Partners create new paths forward after a neurological injury. We provide intensive, individualized services working one-on-one with our patients, and building their skills in their homes and communities. Patients are accepted anywhere on the continuum from severe brain injury to job readiness. Our experienced teams step up to the challenge of building a program that is just right for our patient's specific issues.

Areas of responsibility include but are not limited to:

  • Evaluates the patients’ work and educational history, interest, aptitude, ability and physical skill to vocational potential and necessary training. Develop with interdisciplinary rehabilitation team, vocational rehabilitation treatment plan based on evaluation results and goals patient.
  • Administers direct treatment utilizing vocational rehabilitation techniques. Sets up programs to build the skills necessary to progress persons served toward the highest level of independent functioning. Emphasizes skills related to vocational pursuits.
  • Design pre-vocational program to measure the current skill, ability, and interest patient. Oversee individual program/treatment plan implemented by supported rehabilitative staff.
  • Design community reintegration activities to assist the patient with their vocational needs during physical activity. Assist the patient in understanding their various needs and limitations and how to manage them during function.
  • Provide training to the patient and as appropriate, all others involved in that patient’s rehabilitation. Analyze and document the result of any training.
  • Perform a thorough evaluation, assessing the needs of the patient, and set measurable objectives that are formulated in conjunction with the rehabilitation team. Adjust treatment to achieve maximum results.
Qualifications:
  • Masters Degree in Rehabilitation Counseling or the equivalent plus a minimum of two years vocational counseling experience with individuals with neurological impairments is required.
  • Knowledge of complex neurologic injury and orthopedic rehabilitation techniques generally required.
  • Maintain professional license and/or certification as required by the state in which the therapist practices
  • Good organizational and communication skills are needed.
  • Ability to function with minimum supervision.
  • Reliable transportation is required.
  • The candidate will have the physical ability to perform job related duties which may require lifting, standing, bending, stooping, stretching, walking, pushing, and pulling. A health examination must be successfully completed prior to assignment and periodically thereafter, as required by Company policy, and/or state regulations, to verify that he/she is physically capable of performing assigned duties.
