About Job

JOB REQUIREMENTS: The Social Worker in Adult Protective Services

receives, responds, and investigates reports of abuse, financial

exploitation, and neglect for elders and adults with disabilities;

petitions for guardianships; and reviews court-ordered protective

placements. Duties include: Investigates referrals of individuals

residing in the community who may be at risk of emotional, physical, or

financial abuse or neglect, meeting individuals in their homes Assesses

individuals\' personal and environmental safety, risk, physical and

cognitive capacity, support systems, and vulnerability Respecting rights

to self-determination, identifies and discusses services and supports

and/or interventions to reduce risk Develops and implements safety plans

and arranges for needed services Assesses appropriateness of referrals

for guardianship, reviewing medical records and evaluating competency;

prepares and files petitions for guardianship and protective placement

with Probate Court Attends all court hearings and proceedings; remains

Available To Guardians For Consultation About Options If Circumstances

change Completes annual reviews of protective placements, scheduling

visits with clients in residential facilities and attending court

hearings Participates in other team activities such as assistance with

Powers of Attorney for Health Care and public education on dementia and

abuse prevention Maintains up-to-date notes and files; attends case

reviews and staff meetings Collaborates with other organizations to

improve services for older adults and persons with disabilities Responds

to incoming program calls and processes referrals Performs other work as

required or assigned Experienced social workers with a passion for

working with and advocating for older adults and persons with

QUALIFICATIONS: Experience One year of work experience in human services

or similar field; three years of experience preferred Experience working

with the court system Success Factors Knowledge of: Aging process and

mental health; issues and behaviors associated with dementia, mental

illness, and substance dependence Community resources for elders and

persons with disabilities Wisconsin statutes, specifically Chapters 46,

51, 54, and 55 Legal system and Wisconsin Act 187 Skill in: Proficiency

in Microsoft Word, Outlook, Excel Organization De-escalation techniques

Motivational interviewing Ability to: Make unannounced home visits Write

effectively for evaluations, court documents, and other documentation

Work independently, under pressure, and meet deadlines Work with

medical, mental health and legal professionals, and the courts Remain

professional and courteous at all times Maintain confidential

information Travel approximately 50% of time for home visits in Kenosha

County and for protective placement reviews at various Wisconsin

locations Work respectfully in a diverse and inclusive environment
