Position: Care Coordinator/Care Guide
Responsibilities:
- Deliver high-quality housing stability services, ensuring members receive exceptional care navigation.
- Conduct comprehensive assessments to identify members' housing needs and develop personalized care plans.
- Provide guidance and support to members navigating housing instability, addressing barriers, and facilitating access to resources.
- Collaborate with interdisciplinary teams to ensure coordinated care and support for members.
- Utilize expertise in housing instability to inform program development and service delivery.
- Maintain accurate documentation and data records in compliance with regulatory requirements and timelines.
- Other duties as assigned.
Requirements:
- Bachelor’s degree in social work, social services, or related field.
- Demonstrated expertise in housing instability and related social determinants of health or mental health.
- Proven ability to collaborate effectively.
- Capable of working in both office and remote environments.
- Internet bandwidth roughly 100mbps to ensure stability
- Pacific Time zones- 8-5 PT or 9-6 PT
- Must reside in Southern California
Preferred:
- Master’s degree in social work or related field.
- Experience working with Medicaid, Medicare, and D-SNP health plan members.
