Job details:

• Job title: MSW (Masters of Social Work)
• Shift: Day 5x8-Hour (08:00 - 16:30)

• Duration: 13 weeks
• Weekend Requirements: rotating weekend coverage is expected
• Daily Census: Case Load up to 25 patients

Job requirements:

• 2-year acute hospital case management experience required
• Acute Social Work Experience
• Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, POLST
• Ability to organize and participate in family meetings.
• Participate in the development of the treatment plan.
• Performing both roles of Care Coordination and AIDA, PASRR
• Performing role of care coordination and discharge planning
• Computerized documentation, is required.
• Will be required to watch up to 2 units on the weekend and focus on discharge planning
• BLS
