Job Summary: The full-time Hospice CNA position exists to consistently and compassionately provide high-quality care to Hospice patients in their homes, functioning as a para-professional member of the hospice care team. Performs ADLs and various services for patients as necessary to meet the patient’s personal needs, to promote the patient’s comfort and dignity, and to promote a safe environment. Observe patients and report/document these observations and the services provided. Travel, as necessary.
Qualifications/Educational Requirements:
- Minimum age requirement of 18.
- Completion of a recognized Nurse Assistant certification program (Active CNA license)
- Minimum of one year of direct patient care experience in an institutional or home care setting required. Hospice experience preferred.
- Ability to read, write, and follow instructions and document care given in a computer.
- Understands hospice philosophy and needs of terminally ill.
- Demonstrates compliance with State and Federal regulations.
- Demonstrates tact, patience, strong professional boundaries, and good personal hygiene.
- Excellence in written and oral communication skills.
- Valid driver’s license with good driving record and reliable transportation that is insured in accordance with the state and Dignity Hospice of Colorado’s requirements.
- Satisfactory references from employers and/or professional peers.
- Satisfactory criminal background check.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling all obligations.
Responsibilities/Essential Functions:
- Assists patients, in multiple locations daily, with performing personal care activities (ADLs) as prescribed in the patient’s care plan, including hygiene, mobility, and elimination. If unable to perform a certain task, charts accurately and reports to RN Case Manager/CNA Coordinator immediately.
- Provides supplies to patients (briefs, wipes, hygiene items, etc) as needed to complete plan of care.
- Reports on patient’s condition and significant changes to the RN Case Manager and CNA Coordinator; submits visit documentation in EMR within 24 hours of patient visit.
- Meets safety needs of patients, uses equipment safely and properly; practices safe lifting and transfer techniques, offers ambulation, normal range of motion and positioning techniques in accordance with plan of care; ensures quality of care.
- Performs or assists patient/family in performing home maintenance activities as prescribed in the care plan, including keeping patient's living area clean and orderly (within the scope of the role), supporting independence, providing companionship, and providing respite for families (during the assigned visit).
- Assists patient/family/caregivers with education and teaches EOL care techniques, promoting quality and continuity of care.
- Monitors adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and records/reports appropriately.
- Practices universal precautions in all contact with patients.
- Respects the autonomy and right to confidentiality of all patients and families.
- Identifies the need for intervention by other members of the interdisciplinary team on an ongoing basis and refers/reports to RN Case Manager.
- Provides input into interdisciplinary evaluation of effectiveness of care.
- Manages a caseload and delivers care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
- Participates in pick-up shifts or overtime as necessary to meet patient needs.
- Assures compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
- Attends and participates in team meetings, staff meetings, department meetings, briefings, in-services, committees and other related activities as needed. Contributes positively to patient care planning.
- Completes at least 12 hours of in-services per year by their assigned due date. Attend memorial or funeral services quarterly as applicable.
- Maintains regular attendance.
- Interacts with all staff (Dignity staff and community partner staff) in a positive and motivational fashion supporting the company’s mission.
- Conducts all business activities in a professional and ethical manner.
Working Conditions: General offices, extended care facilities, patient home settings, and exposure to infectious diseases, frequent and extensive travel by automobile.
Physical Demands: Requires physical exertion, including ability to lift 50 lbs. Duties of patient care during transfer activities (adjusting the patient in bed, transferring the patient from bed to wheelchair or vice versa, and chair to toilet, etc.) require that the employee have the physical ability to maneuver patients according to the care required. Additional physical abilities required during these activities will vary according to the level of patient cooperativeness, patient weight, available space, and available equipment. Regularly required to use hands, talk and hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients’ lifestyles.
Continuing Education Requirements: Employee is expected to participate in initial Medicare-required training series AND 12 hours of education per year as assigned. Employees must attend mandatory education programs and trainings.
