LSSI is hiring a full-time Counselor Case Manager, Sunday through Thursday. Responsible for providing intensive community-based case management services for individuals with serious mental illness and substance use to promote independence in the community functioning and decrease psychiatric hospitalizations and crisis episodes. Individuals may be part of the Colbert or Williams Consent Decrees.

Essential Functions

  • Provide and coordinate a continuum of treatment services.
  • Carry a caseload of clients providing supportive counseling and concrete services assigned by supervisor and operate as part of a treatment team.
  • Ensure all documentation is complete in a timely manner and in accordance with Rule 140 requirements, DHS, and the Williams and Colbert Consent Decrees.
  • Meet financial productivity targets.
  • Responsible for transportation of clients in personal vehicle and/or agency vehicle.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.

Position Qualifications

  • Bachelor’s degree in social work, counseling, or related field required.
  • Master’s degree in social work, counseling, or related field preferred.
  • One (1) to three (3) years of clinical experience preferred.
  • Trained and proficient in the use of an electronic health care record required.
  • Experience in Crisis Intervention and De-escalation preferred.
  • Familiarity with accreditation and licensure standards preferred.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Background check clearance required.
  • Valid driver’s license, in good standing for the state of residency required.
  • Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
