About the Role
Join our patient-focused telehealth team and help remove barriers to great virtual care. You’ll guide patients through resource navigation, document essential care-coordination activities in our EMR, and collaborate closely with clinicians—all from the comfort of your home office, with some onsite meet-ups in Winter Park, FL.
What You’ll Do
- Conduct psychosocial check-ins via video or phone and record concise notes in the EMR.
- Connect patients with community, insurance, financial assistance, and care management resources.
- Track preventive-care reminders and follow-ups, escalating complex cases to nursing staff.
- Draft clear, audit-ready summaries that support value-based and insurance billing requirements.
- Facilitate virtual support groups and occasional local outreach events (travel reimbursed).
Who You Are
- Bachelor’s degree (or higher) in Social Work, Human Services, Psychology, or equivalent field.
- Current Florida LSW/LCSW—or eligible for registration—preferred; candidates with strong care-coordination experience in outpatient or home-health settings welcomed.
- Adept at motivational interviewing, resource navigation, and accurate documentation.
- Tech-savvy self-starter who thrives in a flexible, mostly remote environment.
- Bilingual Spanish/English a plus.
