Position Summary: Social Worker
Position Duties: Discharge planning, facilitating treatment team, completing treatment plans, facilitating process groups
Minimum Required Qualifications: LMSW, licensed in PA; 2+ years recent acute hospital social work experience; Experience with discharge planning, facilitating treatment team, completing treatment plans, and facilitating process groups
Length of Assignment: 13 weeks
Shift / Hours Per Week: 8A - 4:30P with every other weekend (40hrs/wk)
Systems: Epic
Start Date: ASAP
