About Job
We are seeking a compassionate hospital social worker to provide emotional support, counseling, and resource connection to patients and families during challenging times.
- Perform social history assessments and develop social services plans.
- Manage crisis situations related to emotional, social, or economic stresses.
- Assist with discharge planning, home evaluations, and follow-up care recommendations.
- Document all activities in patient records and coordinate community resource referrals.
- Support out-of-town families with lodging and transportation arrangements.
- Handle sensitive tasks such as safeguarding property, reporting child abuse, and managing involuntary commitments.
- Obtain necessary consents for care, transfers, and disclosures, ensuring continuity of care.
- Assist patients in understanding their rights and advance directives.
- Requirements include a Bachelor's degree in Social Work or related field, with a Master’s and licensure preferred; at least 2 years inpatient medical behavioral health experience; basic computer skills required. This is a temporary, in-person position.
