We are seeking a compassionate hospital social worker to provide emotional support, counseling, and resource connection to patients and families during challenging times.

• Perform social history assessments and develop social services plans.
• Manage crisis situations related to emotional, social, or economic stresses.
• Assist with discharge planning, home evaluations, and follow-up care recommendations.
• Document all activities in patient records and coordinate community resource referrals.
• Support out-of-town families with lodging and transportation arrangements.
• Handle sensitive tasks such as safeguarding property, reporting child abuse, and managing involuntary commitments.
• Obtain necessary consents for care, transfers, and disclosures, ensuring continuity of care.
• Assist patients in understanding their rights and advance directives.
• Requirements include a Bachelor's degree in Social Work or related field, with a Master’s and licensure preferred; at least 2 years inpatient medical behavioral health experience; basic computer skills required. This is a temporary, in-person position.
