- Assures spiritual assessment of Seniors/clients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
- Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
- Provides direct spiritual care to Seniors/clients and families/caregivers.
- Serves as liaison and support to community chaplains and spiritual counselors.
- Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
- Provides consultation, education, and support to the interdisciplinary group on spiritual care.
- Provides for funeral or memorial services for Seniors/clients as requested.
- Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
- Drives from one Senior/client to another, adapting to changing road and/or weather conditions.
COMMUNICATION: Must be able to speak, read, write, and understand the English language including terminology related to the job.
WORKSITE CONDITIONS: Sometimes works indoors in a facility maintained by Tabitha that will typically be in good repair, clean, well ventilated and well lit. Frequently works in Senior/client homes which will vary as far as their state of repair. Frequent exposure to various road and driving conditions which are subject to change without notice.
PHYSICAL REQUIREMENTS: Ability to drive vehicle from place to place, gain access to each Senior/client home and then safely perform the duties of the job. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability of 25 - pounds, demonstrating strength and agility at Tabitha’s discretion.

AGE REQUIREMENT: At least age 18.
EDUCATION: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
EXPERIENCE: Hospice experience preferred. Experience working with death and dying, individuals/family/caregiver preferred.
CREDENTIALS: CPR required or must obtain within 30 days of employment. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.
