Alliance Health Centers is a Federally Qualified Health Center Look-Alike (FQHC-LAL) dedicated to providing high-quality, comprehensive healthcare services to underserved communities in Fort Wayne, IN. We focus on integrated care that includes primary care, behavioral health, substance use treatment, and support services for individuals facing homelessness, disabilities, and other barriers to healthcare.
Position Overview:
We are seeking a compassionate and dedicated Licensed Mental Health Counselor to join our Behavioral Health team. The counselor will provide trauma-informed mental health services, including individual and group therapy, to a diverse population, including individuals experiencing homelessness, those affected by substance use disorders, and patients with disabilities. This role involves collaborating with medical providers, case managers, and other community partners to ensure comprehensive care for our patients.
Key Responsibilities:
- Provide individual, group, and family counseling using evidence-based practices.
- Conduct comprehensive mental health assessments and develop individualized treatment plans.
- Utilize trauma-informed care approaches, such as Eye Movement Desensitization and Reprocessing (EMDR) and flash therapies, to support patients with a history of trauma.
- Collaborate with primary care providers, substance use treatment teams, and other members of the interdisciplinary team to coordinate holistic patient care.
- Offer telehealth services for mental health and substance use treatment as needed.
- Maintain accurate and up-to-date clinical documentation in compliance with HIPAA and organizational standards.
- Participate in case management and care coordination meetings to ensure continuity of care for patients.
- Provide crisis intervention services as needed, including risk assessments and safety planning.
- Stay current on best practices, emerging trends, and continuing education relevant to the mental health field.
- Engage in outreach activities to connect with underserved populations, including street medicine visits to homeless encampments.
Qualifications:
- Required: Master’s degree in Counseling, Social Work, or a related field from an accredited program.
- Licensure: Must hold an active Indiana state license as an LMHC, LCSW, or equivalent.
- Experience: Minimum of 2 years of experience in a clinical mental health setting, preferably working with underserved or high-risk populations.
- Skills: Proficiency in trauma-informed therapies (e.g., EMDR, flash therapy) is preferred.
- Strong communication skills and the ability to build rapport with diverse populations.
- Familiarity with telehealth platforms and electronic health record (EHR) systems.
