  • Collaborate with clients to develop a housing plan that includes short- and long-term housing goals and connections to supportive services that address safety, stability, income, and overall wellness.

2. Housing Identification & Landlord Collaboration:

  • Coordinate with the GNRRH Outreach Specialist to assist clients in locating and securing safe, decent, and affordable housing.
  • Facilitate lease-up, move-in, and rental assistance processes in coordination with the GNRRH Director and Outreach Specialist.

3. Ongoing Case Management & Support:

  • Meet with clients regularly through home visits and/or check-ins to monitor housing plan progress, troubleshoot obstacles, provide client-centered supports, and ensure housing retention.
  • Provide assistance and resources for life skills, budgeting, tenant rights, public benefits, and employment services.
  • Coordinate access to health, mental health, legal, childcare, and substance use resources.

4. Documentation & Data Compliance:

  • Maintain accurate, HUD-compliant documentation in a secure database.
  • Monitor housing outcomes, service delivery, and client progress for reporting and evaluation.
  • Support data entry, internal audits, and reporting for grant compliance.
