About Job

Job Description:

As a Level of Care Coordinator, you will be responsible for the assessment, referral and monitoring of behavioral health and substance abuse services for youth involved in the Juvenile Probation System and the Child Welfare System.

  • Meets with identified children and adolescents in shelter or as otherwise assigned by the supervisor to implement a Level of Care assessment to assess the appropriate service need and level.
  • Prepares written summaries within a specified time frame documenting assessment results and recommended treatment and/or services.
  • Routes completed summaries to designated Child Welfare Case Workers or Juvenile Probation Officers and for utilization in planning for court appearances and subsequent placement or treatment decisions regarding the youth assessed.
  • Convenes or participates in related Child and Family Inter-agency Team meetings, ensuring the participation of families and team members.
  • Develops or participates in the development of clinically appropriate plans, focusing on robust discharge plans.
  • Participates and provides consultation to the Triage Team for both the Child Welfare and the Juvenile Probation systems.
  • Secures and/or completes all required paperwork for referrals for BHRS, RTF, MST or other community services.
  • Provides MH BSU representation at Inter-agency Team meetings, as indicated.
  • Adheres to CASSP principles and commitment to cultural competence in all aspects of assessment and service planning, ensuring that each child is maintained in his/her own home and/or community wherever possible.
  • Adheres to ECCM confidentiality and HIPAA policies and procedures.
  • Maintains effective working relationships with OCY Case Workers, Juvenile Probation Officers, juvenile court judges and detention and shelter staff.
  • Participates in weekly supervisory and compliance review meetings, as required.
  • Collects data and submits to the identified database via established protocol.
  • Needs to be comfortable meeting with parents/providers to get info for the assessment.
  • Refer for psychiatric assessments, psychological, and psycho sexual evaluations.
  • Participation in peer jury and district magistrate proceedings (diversion).
  • Other duties, as assigned by the responsible Administrative Officer.

Qualifications:

Master’s Degree in a mental health-related field, as well as Licensure such as LSW, Licensed Psychologist, LPC preferable
