Care Coordinator - Position Overview

  • The Care Coordinator is responsible for facilitating referral and engagement into the Canopy Carenet.
  • The Care Coordinator facilitate workflows and processes which includes assessing service needs, and managing the request for care within Canopy’s Carenet.
  • Within the behavioral health clinics, the Care Coordinator assist with scheduling as needed, coordination of insurance benefits as needed and provide linkage to a range of social, educational, vocational, recreational, and other related wrap around service needs identified through targeted case management follow up.
  • The Care Coordinator works collaboratively as part of the admissions team and with clinical staff to create a supportive environment to address the basic needs of the child and family through referral, service, education and skill development.

Job Responsibilities

  • Manages incoming calls and process calls related to care request.
  • Routes calls to the appropriate Solution staff to address needs.
  • Assist in the coordination of care which includes assisting in identifying and assessing care and resource needs for children, youth, and their families.
  • Assess individualized services needs of the family and documents in the Organization’s electronic documentation system.
  • Facilitates access to medical, social, educational, vocational and other needed services internally and externally as indicated for coordination of care needs.
  • Coordinates with the Accounting Department to resolve conflicts in data input related to the coordination of services as indicated and applicable for billable services provided through care coordination.
  • Documents service delivery and communication in the Organization's electronic health record system.
  • Ensures documentation adheres to standards and regulations for Organization requirements and if applicable, regulatory agencies.
  • Assesses service needs for referrals and active cases and provide effective caseload management through engagement activities designed to support treatment participation and support clinical services delivered in the behavioral health clinic setting as needed and identified.
  • Demonstrates ability to monitor treatment progress in service coordination and document engagement coordination accordingly.
  • Demonstrates the knowledge to facilitate the process to gauge customer and referral satisfaction through outlined engagement and marketing strategies.
  • Conduct telephone support to families for engagement sessions to determine barriers to treatment and compliance.
  • Create and develop triggers for discharge plans for children and youth disengaged in care in the Organization's electronic health record system.
  • Initiates the ten-day letter process to help reinforce the missed appointments and noncompliance policy within the behavioral health clinic setting.
  • Conduct daily review of the behavioral health clinic appointment schedule to initiate the strategies for success protocol to promote engagement and help reduce no-show rates as applicable.
  • Review and monitor treatment plans and the progress in the care journey for cases active in targeted case management to evaluate compliance.
  • Screen and evaluate requests for services.
  • Work with interdisciplinary team in reviewing cases and developing strategies to reengage children and youth into services within the behavioral health clinics.
  • Adheres to identified work schedule set by the Director of Admissions and/or Leadership.
  • Prepares and completes program reports as assigned by Director of Admissions and/or Leadership.
  • Attends staff meetings as requested and scheduled.
  • Promotes the Organization’s solutions and Carenet services to the public.
  • Promotes the Organization's Core Values and Mission.
  • And all other duties and/or responsibilities as assigned.
  • Meets all Key Performance Indicators (KPIs).

Required Qualifications

  • Bachelor’s degree in social work and licensed- LSW or Master’s degree in counseling, social work, psychology, health services, or social rehabilitative services or related field required.
  • Must either be licensed or certified through the Department of Mental Health as a PCMHT or CMHT.
  • Must have at least one year of mental health experience.
  • Must be at least 21 years of age, have a valid driver license, a good driving record, proof of legally appropriate auto liability insurance, and must be willing to use personal vehicle for organization business.
