Now Hiring: RN Care Manager (2 days onsite, 3 days remote) | Seattle, WA 98144

Join our team as a Registered Nurse - Care Manager in a dynamic hybrid role based out of Seattle, WA — offering both on-site and remote flexibility. This 13-week contract offers 5x8 hour day shifts (8:00 AM – 4:00 PM, Monday to Friday) and is open to both local and non-local candidates.

Position Details:

• Job Title: Registered Nurse - Care Manager
• Location: Seattle, WA 98144 (2 days onsite, 3 days remote)
• Shift: Days – 5x8 (Monday to Friday)
• Contract Length: 13 weeks
• Start Date: 08/25/2025
• Schedule Type: Self-scheduling
• Guaranteed Hours: 40 hrs/week
• Charting System: Epic (Required)
• Pay Package: $2550 - $2750/week

Requirements:

• Minimum 1 year of RN experience in acute care, LTAC, or rehab/skilled nursing
• BLS Certification (Required)
• Experience in care coordination, discharge planning, utilizing InterQual/Milliman guidelines
• Reliable transportation (no public transit)
• Open to temp-to-perm opportunities

Preferred Skills:

• Familiarity with CMS, DRG, ICD-10, NCQA, HEDIS, HIPAA, OSHA
• Experience with population health, transitional care, and reducing hospital readmissions

Work Setting:

• Outpatient Clinic
• Patient population includes adolescents, adults, and geriatrics
• Focus on ED visits, IP discharges (excluding L&D and oncology)
• Daily patient load: ~8 visits
• Interdisciplinary collaboration with Social Services
• Scrub Color: Business casual for remote days, clinical attire on-site
