Job Opportunity:

The Associate Behavioral Health Specialist (AMFT, ACSW, or APCC) is a member of the Behavioral Health team and is responsible for providing behavioral health care for individuals, couples, families and groups for patients.

Hiring for (2) full-time, in-person openings seeing patients at (2) of our health center locations in Guerneville, Occidental, and/or Sebastopol, CA.

Duties:

  • Work with Medical, Behavioral Health and other staff to provide assessment, intervention, and brief and ongoing behavioral health services, including psychotherapy, to patients with medical and behavioral health concerns within the primary care setting.
  • Participate in care coordination, patient education, and information and referral.
  • Provide facilitation of groups for individuals with similar diagnosis and symptoms.
  • Provide crisis assessment and intervention and be available for warm hand-offs from primary care staff.
  • Focus upon prevention and early intervention from a multi-generational approach with a focus upon social determinants of health and trauma-informed practices.
  • You will invest in short and long-term relationships with a diverse group of patients over time to develop a trusting, healing environment for patients to thrive.
  • Participate in case consultation, peer review, team meetings and other team activities.
  • Participate in supervision requirements including meetings with clinical supervisor and documentation guidelines per clinical supervisor.

Minimum Requirements:

  • Completed graduate degree in Social Work, Counseling, or other applicable degree.
  • Associate Registration with the Board of Behavioral Sciences as an AMFT, ACSW, or APCC.

Experience & Qualifications:

  • Experience working with diverse populations providing individual, family and couple’s counseling.
  • Experience in health care settings and behavioral health care preferred.
  • Bilingual/bicultural in Spanish strongly desired.

Physical Requirements:

  • Must be able to move freely about the office and carry light loads (up to 25 pounds).
  • Prolonged standing and occasional sitting required.
  • Ability to work with office equipment, including computers.

Pay: $35.00 per hour
