HOUSING CASE MANAGER - (Part Time)
The Part Time Housing Case Manager supports the mission of DAP by working with families experiencing homelessness or housing insecurity due to domestic violence. This position is responsible for providing intensive case management services to help clients obtain and maintain stable, secure housing through the connection of resources including Rapid Rehousing, Relocation Assistance and other housing related resources. The Housing Case Manager works with clients to develop individualized housing plans that support measures towards permanent housing and the over-all goal of self -sufficiency. This position is a part-time (24 hours per week) position.

Essential Functions:
  • Provides ongoing intense case management that includes home visits & supports housing goals
  • Completes housing assessments
  • Completes relocation applications
  • Works collaboratively across programs and organizations to serve the housing and financial sustainability needs of clients
  • Provides referral linkages to external supports
  • Provides training and education related to housing assistance and resources
  • Provides financial literacy education
  • Surveys rental markets for affordable housing options
  • Assists with the promotion of housing programs to interested landlords and property managers
  • Advocates for clients with prospective landlords
  • Facilitates negotiation and resolution of rental contracts and/or rental arrears
  • Processes housing program applications including Rapid Rehousing and Relocation Assistance
  • Assists clients with completing supportive and subsidized housing applications
  • Conducts HUD mandated HQS inspections
  • Conducts at home visits in accordance with program requirements
  • Provides individuals in crisis with appropriate interventions
  • Accurately inputs statistical information in DAP’s database and other required data systems
  • Works cooperatively with Safe House staff to support clients housing goals
  • Works cooperatively with other DAP staff to ensure holistic response & service delivery to clients
  • Assists Associate Director of Housing Services with funder reports and monitoring
  • Ensures all data entry is up to date in database and all statistical forms used by Housing program
  • Ensure files/paperwork comply with funder requirements by performing monthly internal audits
  • Attends all Housing case consultation meetings
  • Maintains records of all referrals made to Housing program and ensure referrals are being completed by assigned staff
  • Completes monthly program reports as requested by supervisor
  • Participates in program and organizational staff meetings
  • Accurately complete and submit time sheets on time and in accordance with DAP’s policies and procedures
  • Fulfill training requirements on a timely basis and in accordance with DAP, PCADV & PCCD policies
  • Performs other duties as assigned
Qualifications and Competencies:
  • Experience in social service setting with working knowledge of case management systems and planning techniques
  • Knowledge of housing related resources, housing rights issues, and the continuum of care for homeless persons
  • Experience working with homeless individuals and families
  • Demonstrated proficiency in Microsoft Office Suite and experienced in using computer based database systems.
  • Willing and able to confidently travel as needed throughout Delaware County, PA
Work Environment:
  • Work is primarily performed in the field within Delaware County
  • Work requires routine home visit to client’s homes and on-site rental unit inspections
  • Work requires visits to landlords and property managers
Other Requirements:
  • Successful completion of required organizational training within 2 weeks of hire
  • Satisfactory Criminal Background Check and other organizationally required clearances
  • Must have valid drivers license, valid vehicle insurance and reliable transportation
