About the role
As a Care Manager you will support patients both virtually and in-person by providing PAP therapy instructions, support for mask fit and pressure comfort to drive increased PAP therapy adherence support.
The ideal candidate will be flexible and possess the ability to make sound judgments.
What you'll do
- Instruct patients on proper use, maintenance, and cleaning of sleep apnea equipment
- Discuss sleep test results with patients and provide them with information regarding treatment alternatives based on their physician's recommendation.
- Communicate with outside Respiratory/DME providers to facilitate patients' sleep equipment needs.
- Monitor and maintain PAP compliance documentation in the EMR system and other designated databases and portals.
- Perform patient outreach phone calls at prescribed time periods
- Help patients to troubleshoot and remove barriers for effective PAP use, and serve as a clinical liaison to physicians regarding issues associated with patient PAP compliance.
- Role requires knowledge of insurance payer reimbursement and adherence guidelines for Obstructive Sleep Apnea therapy.
- Scheduling patients for support services as needed in the operating system.
- Communicates financial liability to non-compliant patients based on insurance payer guidelines.
- Support PAP Supply Technicians as needed.
Qualifications
Essential:
- High School Diploma
- Minimum 2-3 years, clinical experience as certified Clinical Medical Assistant in Sleep Medicine and/or relevant specialty
- Demonstrates excellent written and verbal skills
- Excellent time management, organization skills, comfortable and professional when communicating via phone, email and text
- Ability to speak clearly and effectively over the phone to patients, other physician practices, and employees of the organization.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Desirable:
- Strong computer skills including: Word, Excel, CareCloud
- Knowledge of PAP devices and supplies
- Accustomed to working in a high volume, fast paced, goal oriented environment.
- Team player that recognizes value in nurturing relationships with different departments
- Experience working with physicians
