- Conducting thorough assessments of patients' social, emotional, medical, spiritual, and physical needs to understand their specific circumstances
- Developing individualized discharge plans that address mental health and substance use needs, ensuring continuity of care
- Connecting patients with appropriate post-discharge resources, such as outpatient therapy, support groups, housing, and vocational rehabilitation
- Working with healthcare teams, community resources, and other relevant parties to facilitate a seamless transition to the next level of care
- Maintaining accurate and up-to-date records of discharge plans, client progress, and referrals
- Collaborating with healthcare professionals, social workers, and community organizations to ensure seamless transitions and continuity of care
- Maintaining accurate and comprehensive records of patient interactions, discharge plans, and follow-up activities
- Providing information and support to inmates and their families regarding discharge planning, available resources, and post-release care options
- Acting as a liaison between the correctional facility and community agencies to facilitate referrals and access to services
- Assist patients with a specific plan for medication management
- Assist patients with the identification of a support network, instructions for future care and follow-up
- Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
- Bachelor’s Degree in a human service field bachelor's degree in a related field like nursing, social work, psychology or healthcare
- Possess an active CPR certification
- Knowledge in the development of discharge plans and coordination with community service providers
- Ability to maintain confidentiality of all proprietary and/or confidential information
- Excellent communication skills are essential for interacting with patients, staff, and physicians
- Able to work independently and in a team
- Ability to analyze data and solve problems independently.
- Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements
