Job Summary:
The Care Coordinator is responsible for ensuring high-quality home care services by overseeing the coordination, implementation, and evaluation of individualized care plans for clients. This role serves as the primary liaison between clients, families, caregivers, and healthcare professionals to ensure that all care needs are met with compassion, professionalism, and compliance.
Key Responsibilities:
· Conduct initial client assessments and home visits to determine care needs and preferences
· Develop and update individualized care plans in collaboration with clients, families, and providers
· Schedule and assign caregivers based on client needs, preferences, and staff availability
· Monitor the quality of care provided through regular check-ins, supervision, and performance reviews
· Maintain accurate documentation of client records, visits, updates, and incidents
· Communicate with caregivers to provide guidance, training, and support
· Serve as the main point of contact for client and caregiver concerns or emergencies
· Coordinate with physicians, case managers, and insurance companies as needed
· Participate in on-call rotation for after-hours client support
· Ensure compliance with HIPAA, agency policies, and state/federal home care regulations
Qualifications:
· High school diploma or equivalent required; associate or bachelor’s degree in healthcare or human services preferred
· Minimum of 2 years of experience in home care, healthcare, or case management
· Strong understanding of caregiving procedures, scheduling, and client relations
· Excellent communication (both verbal and written) organization, and time-management skills
· Ability to work independently and as part of a team
· Proficiency in Microsoft Office and care coordination software (e.g., ClearCare, AlayaCare, etc.)
· Valid driver’s license and reliable transportation

Job Type: Part-time
Pay: $19.00 - $20.00 per hour
Expected hours: 20 – 25 per week
