Position Overview:
As a Social Worker, you will play a vital role in supporting our residents' social, emotional, and psychological well-being. You will assist in their adjustment to our facility, help develop comprehensive care plans, and work closely with families to ensure a smooth transition and discharge process.
Key Responsibilities:
- Assist residents in adjusting to the facility and navigating room changes.
- Provide emotional and social support to residents and families.
- Facilitate family group meetings and assist with personal needs.
- Collect social history data and assist in discharge planning.
- Complete and maintain documentation, including MDS and Care Plans.
- Review and educate residents and staff on residents’ rights.
- Assist families with engraving or marking personal belongings.
- Participate in process improvement initiatives to enhance resident care and workflow.
- Support special projects and perform additional duties as assigned.
Work Environment:
- Primarily indoors within a long-term care facility, including resident rooms.
- Work among healthcare staff, residents, and guests.
- Potential exposure to:
- Slippery floors, chemicals, and hazardous materials.
- Bloodborne pathogens and communicable diseases.
- High-stress medical or life-threatening situations.
