IBH Care Coordinator (Integrated Behavioral Health)
Location: San Leandro, CA
Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.
Salary Range: $66,000 – $80,000, depending on experience
Job Summary
This position supports integrated behavioral health efforts by connecting individuals to behavioral, medical, and social services. It’s a great opportunity for an Associate Marriage and Family Therapist seeking meaningful clinical experience within a hybrid care coordination model serving underserved communities in Alameda County. Clinical supervision is available.
Core Duties
- Deliver client-centered coordination through in-person and telehealth services
- Complete behavioral health assessments and contribute to treatment plans
- Guide clients through referrals and transitions across service systems
- Ensure consistent follow-up to support engagement
- Collaborate with a broad network of providers and support systems
- Maintain accurate records using an EHR
- Track service metrics and assist in reporting outcomes
Requirements
- ASW/ACSW license in California
- Master's Degree in social work, behavioral health, or public health
- Minimum 1 year of behavioral health or social services experience
- Working knowledge of Alameda County service systems
- Organized, responsive, and skilled at interdisciplinary teamwork
- Proficiency in documentation standards and HIPAA guidelines
Preferred
- Familiarity with integrated or primary care
- Multilingual skills (Spanish preferred)
- Valid California driver’s license
