Overview:
Sentara Health Plans is hiring a Temporary Care Coordinator for the Western/Roanoke area!
Position Status: This temporary position is expected to last a minimum of 3 months with the possibility of extension.
Position Location: Hybrid in Western/Roanoke area. Must be able to travel 3-4 days a week for in-person assessments.
Area includes: Amherst, Bedford, Appomattox, Palmyra, Prince Edward, Campbell, Nelson, Botetourt
This position requires both in-person face-to-face assessments and remote telephonic assessments! Applicants must be located in the Western/Roanoke area.
Standard Working Hours: 8:00AM to 5:00PM
Overview
Performs a variety of casework duties and provides case management services to patients, families, and caregivers. Develops, participates in, and monitors multidisciplinary collaboration of services to patients when appropriate. Assists adult patients and their families with personal and environmental difficulties related to medical conditions, including terminal illnesses.
Education
- Must possess a degree in Health & Human Services or related fields such as Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation. All degrees must be from accredited schools listed on the U.S. Department of Education College Accreditation database. Degrees from schools outside the U.S. will be reviewed individually.
Certification/Licensure
- No specific certification or licensure required
Experience
- Minimum 1 year of Long Term Care and Health Plans experience required
- Case Management/Care Coordination experience strongly preferred
- Experience with Community Health Resources preferred
- Experience initiating care plans and conducting assessments both subjectively and objectively
