Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role requires a strong commitment to enhancing the well-being of clients through effective intervention strategies, advocacy, and resource management. The Social Worker will work collaboratively with healthcare professionals, community organizations, and families to ensure comprehensive care and support.

Responsibilities

  • Conduct thorough patient assessments to identify needs and develop appropriate care plans.
  • Provide crisis management support to clients in urgent situations.
  • Utilize motivational interviewing techniques to encourage positive behavioral changes in clients.
  • Maintain accurate medical documentation and manage medical records in compliance with regulations.
  • Collect data for ongoing evaluation of client progress and program effectiveness.
  • Perform diagnostic evaluations to determine the appropriate interventions for clients.
  • Collaborate with interdisciplinary teams to coordinate care and resources for patients.
  • Advocate for clients' rights and access to necessary services within the community.
  • Engage with pediatric and geriatric populations, tailoring interventions to meet their unique needs.
