Services Area/Programs: Intensive Case Management
Description of Duties: The individual in this role will serve as Case Manager of the Intensive Case Management Team. The focus of this program is to provide community rehabilitative services that provide direct treatment and restorative interventions as well as case management. The ICM Team is dedicated to supporting clients in reducing psychiatric or substance use symptoms, promoting long-term symptom stability, and restoring essential community living and interpersonal skills. The team also provides immediate crisis intervention to de-escalate urgent situations and ensures timely connection to appropriate community services and resources. Responsibilities of the Case Manager include: Effectively manage a caseload of 15-30 individuals, completing the minimum requirements of no less than (4) face-to-face contacts per month in the individuals' home or choice of a safe community setting. Keep detailed progress notes of visits to home, community locations, and phone contacts. Complete all documentation in the electronic client database within 24-48 hours of service delivery. Meet productivity of no less than 55% or higher each month. Add goals/objectives to Plans of Care that are person-centered according to DBHDD guidelines. Provide follow-up and referral services for individuals as needed. Provide community resources and contacts to the individuals to facilitate delivery of supported housing and employment services as needed. Serve as liaison between individuals, their physicians, and other organizational components to ensure communication and linkage for continuity of care and services. Provide on-call assistance involving rotating on-call duties with team members. Transporting individuals served is a requirement.
Minimum Qualifications: Bachelor’s Degree in psychology or related field. Must have a valid, unrestricted Georgia Driver’s License. Must be keyboard proficient.
Preferred Qualifications: One to two years of experience working with individuals with behavioral and substance use disorders. Master’s degree in a related field.
