Job Summary
The Care Coordinator plays a vital role in ensuring the highest quality of care for residents in assisted living and nursing home settings. This position involves supervising care staff, coordinating resident care plans, and advocating for resident rights. The ideal candidate will have a strong background in senior care, social work, or nursing, with a focus on medication administration and long-term care practices.

Responsibilities

  • Supervise and support care staff to ensure compliance with care plans and resident needs.
  • Develop and implement individualized care plans in collaboration with healthcare professionals and families.
  • Monitor medication administration to ensure accuracy and adherence to prescribed protocols.
  • Advocate for resident rights and promote a culture of dignity and respect within the facility.
  • Conduct regular assessments of residents’ health and well-being, adjusting care plans as necessary.
  • Manage budgeting for care services, ensuring efficient use of resources while maintaining quality standards.
  • Facilitate communication between residents, families, and healthcare providers to ensure comprehensive care delivery.
  • Provide training and support to staff on best practices in senior care and regulatory compliance.

Qualifications

  • Experience in supervising staff within a nursing home or assisted living environment is preferred.
  • Knowledge of medication administration protocols and long-term care regulations.
  • Background in social work or related fields is advantageous.
  • Strong understanding of resident rights and advocacy principles.
  • Excellent organizational skills with the ability to manage multiple priorities effectively.
  • Strong interpersonal skills to foster positive relationships with residents, families, and team members.
  • Ability to work collaboratively within a multidisciplinary team to enhance resident care.
