Our Delaware County program is seeking a dedicated Primary Counselor. The Primary Counselor provides guidance and support to all clients; teach and reinforce skill development in the areas of mental health symptom reduction, daily living skills, routine medical care, and personal hygiene


The Primary Counselor also participate in the development of individual service plans for assigned clients.

  • Conducts a weekly counseling session with all assigned clients to review progress toward treatment plan goals. Successful completion of medication training.
  • Maintains and monitors medications consistent with individuals' self-medication plans, in compliance with State and Agency medication administration regulations.
· Tracks all client progress, as identified in their individual service plans. Provide supplemental reports on individual growth and development, as required. · Record their progress as they move through the service delivery system.
  • Schedules and participates in treatment team meetings and case conferences to discuss progress, needs and concerns of clients.
  • Interfaces with other members of the support team, which may include vocational/educational instruction staff, consultants, case managers, members of the medical profession and family members, as needed.
  • Participates in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.

Job Qualifications:

A successful applicant will have the following:
  • A.A. degree in the social sciences or health care related field and one year of relevant experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a team.

Scope:
  • Requires the ability to lift carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents.
  • Possess manual dexterity and fine motor skills.
  • May be required to possess a valid drivers' license and an acceptable driving record (contingent upon facility location and client and program needs).
  • May require crisis intervention in emergency situations.
  • May be expected to report to work as needed during emergencies including inclement weather.
