Job Summary:

Innerlight Community Advocates is seeking a skilled and compassionate East Houston area QMHP-CS to provide case management and skills training services. This role involves working directly with minor and adult clients in their homes, schools, or other community settings to deliver trauma-informed and client-centered care. The ideal candidate will have a strong background in providing wraparound mental health services, with a commitment to empowering individuals to achieve independence and wellness. Candidate must be able to provide in-person case management services in East Houston communities, such as Baytown, Pasadena, Jacinto City, and others.

Key Responsibilities:

  • Deliver case management services, including resource coordination and referrals for housing, employment, and other social services.
  • Provide skills training to clients, focusing on coping strategies, emotional regulation, and life skills to support long-term recovery.
  • Collaborate with other providers, including therapists, psychiatrists, and case managers, to ensure holistic care for clients.
  • Maintain timely and accurate documentation of services provided in accordance with state and organizational guidelines.
  • Participate in multidisciplinary team meetings to monitor and adjust client care plans.

Qualifications:

  • Eligibility as a Qualified Mental Health Professional-Community Services (QMHP-CS) in Texas.
  • Bachelor’s degree in counseling, social work, psychology, or a related field (Master’s degree preferred).
  • Knowledge of trauma-informed care and evidence-based practices for treating mental health disorders.
  • Strong interpersonal and communication skills with the ability to build trust with diverse client populations.
  • Proficiency in clinical documentation and record-keeping.
  • Valid driver’s license and reliable transportation for community-based visits.

Preferred Skills:.

  • Experience working with Medicaid populations or underserved communities.
  • Familiarity with co-occurring mental health and substance use disorders.
