We are currently seeking a Patient Coordinator to serve as the boots-on-the-ground representative of our agency—working closely with clients, caregivers, and leadership to ensure smooth operations and exceptional service delivery.

Key Responsibilities:

  • Serve as the primary point of contact for clients and their families
  • Coordinate new patient onboarding and ensure seamless start-of-care
  • Provide EVV (Electronic Visit Verification) training and ongoing support for caregivers
  • Monitor and maintain compliance with state regulations and agency policies
  • Assist with caregiver scheduling, patient matching, and service adjustments
  • Support recruitment and community outreach initiatives
  • Act as a liaison between caregivers, patients, and corporate leadership to maintain high standards of care

Qualifications:

  • Previous experience in home care, healthcare coordination, or patient services (preferred)
  • Knowledge of EVV systems and compliance regulations
  • Strong communication and problem-solving skills
  • Ability to work independently while collaborating with agency leadership
  • Proficient in Microsoft Office and scheduling systems
  • Valid driver’s license and reliable transportation
