We are seeking an enthusiastic, hands-on intake social worker to support the intake process and clinical needs of the PHP. The intake social worker will work within a multidisciplinary team in a new, innovative partial hospital setting with adolescents. The intake social worker will lead the intake process of new patients/families enrolling into ilearn wellness PHP.

Duties and Responsibilities:

• Initiates contact to gather required clinical and demographic data from patient and referral sources in a timely manner.
• Reports to clinical director and psychiatrists with clinical presentations from referral sources to streamline intake process.
• Gathers and communicates clinically necessary information for initial pre-authorization as well as ongoing review to utilization review team.
• Collaborates with interdisciplinary team to gather progress in treatment on patients for continued assessment of appropriate level of care.
• Collaborates with appropriate departments to process electronic billing.
• Schedules intakes appointments for patients, greet patients and families as they enter and leave ilearn wellness.
• Assists in milieu management and facilitates therapy groups when needed.

Qualifications:

• Master’s degree in social work, mental health counseling (LCSW, LICSW, or LMHC)
• 3 years of experience in mental health setting including clinical placement
• Preferred experience with adolescent population
• Experience with EMR systems
• Experience and understanding of appropriate LOC information
• Experience with Utilization Review process
