Responsibilities
- Collaborate with all departments to promote and protect residents rights and well-being.
- Demonstrates critical thinking in the identification of social and safety issues that may during the course of residence. Demonstrates initiative and seeks formal and informal opportunities to improve practice.
- Develop and implement written, interdisciplinary care plans for residents from admission to discharge.
- Organize and facilitate meetings for residents' families to improve residence overall well-being.
Required Qualifications
- Social Work Degree, Bachelor's (required) or Master's (preferred)
- Prior experience in long-term care required
- Experience completing PSRR and 408s
- Experience with care planning and discharge planning
- Possess effective team problem-solving and interpersonal communication skills
