Responsibilities

• Collaborate with all departments to promote and protect residents rights and well-being.
• Demonstrates critical thinking in the identification of social and safety issues that may during the course of residence. Demonstrates initiative and seeks formal and informal opportunities to improve practice.
• Develop and implement written, interdisciplinary care plans for residents from admission to discharge.
• Organize and facilitate meetings for residents' families to improve residence overall well-being.

Required Qualifications

• Social Work Degree, Bachelor's (required) or Master's (preferred)
• Prior experience in long-term care required
• Experience completing PSRR and 408s
• Experience with care planning and discharge planning
• Possess effective team problem-solving and interpersonal communication skills
