We have an opening for Chronic Care Manager, LPN, in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.

Our focus is in assisted living, memory care communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical and psych needs.

Being a mobile practice we go out to Assisted Living and Memory Care units to provide services. We usually go to 1 community each day. We are looking for the right person to help us open our new practice in your area. The prospect facilities are located in south Champaign, Danville and Arcola.

My Mobile MD manages both medical and psych services. We have a full time Chronic Care Manager, LPN, covering each community and a full time NP that is usually over 2-3 communities.

This position offers a lot of flexibility and the opportunity to run your own show!

It is a very exciting practice that we have built over the past 10 years. We have successful practices in both Indiana and Ohio and are opening new practices in Illinois and West Virginia this year. We are the preferred provider or medical director in each community we serve. Our reputation in the industry is vastly growing and all we need is you!
